3050 USD - cost of services of the center including selection of a candidature of a surrogate mother, granting of surrogate mothers database, psychologist decision, complex medical inspection of a surrogate mother in a clinic for human reproduction, certificate of absence of previous charge of a surrogate mother, consent of the husband for his wifes participation in the program as a surrogate mother, legal support of the program up to the conclusion of the contract between the surrogate mother and the genetic parents (Customers), translator services (if necessary), guest services (hotel and transport costs are paid additionally);
10000 USD - compensation to a surrogate mother for all the term of pregnancy (payment in stages 10% on the fact of pregnancy approach, 90% 2 days prior to the birth of a child; money are transferred to the surrogate mother settlement account);
1000 USD - bonus to be paid to a surrogate mother;
300 $ X 9 = 2700 USD - the monthly allowance for 9 months of pregnancy is paid per month;
1000 USD - cost of maternity clothes for a surrogate mother for the whole pregnancy period;
3585 USD - one attempt of IVF or ICSI procedure (if a genetic mother makes use of her own genetic matter), the ovulation stimulation madicines should be paid for separately;
7012 USD - one attempt of procedure IVF, cost of service of the donor Ovum and full cost of services of clinic, including all necessary medicines for stimulation of the ovum donor and preparation of surrogate mother's endometrium;
500 USD - medical insurance of substitute mum upon accidents;
1500 USD - Official registration of papers on the child, including the certificate on a birth of the child and all documents for the further departure limits of Ukraine, the delivery of the documents to the embassy shall be made by the genetic parents personally;
800 USD - donors compensation;
1000 USD - cost of services of the center including medical inspection and support up to a puncture Ovum donor;
450 USD - Cost of curator services support and control during 9 months of pregnancy course (full or partial):
12,5 EUR per hour. The partial control hourly visiting the substitute mother, control of compliance with agreement obligations, meals control, accompanying of the surrogate mother to a doctor;
25 EUR per day. The full control is around- the-clock residence together with a surrogate mother, performance of homework, accompaniment during visits to the doctor and the full report with work statement; (separate apartment is granted);
200 USD - Cost of apartment for one month.

The choice of a surrogate mother and egg donor can be made in the cource of introductory consultation on the first day of the visit to the Center.

The package of services for human reproduction clinics includes:

Selection of a surrogate mother candidature from the database of Surrogate Motherhood Center "La Vita Felice";
Psychological diagnostics of surrogate mother;
Medical diagnostics of surrogate mother;
Presentation of surrogate mother candidature completely suitable for the participation in the surrogate motherhood program to human reproduction clinic;
Selection of ova donor.

"Economical" package deal – 3050 USD:

Presentation of candidature of surrogate mother from database of surrogate motherhood center, including the medical inspection of surrogate mother stipulated by the medical law of Ukraine
Psychological diagnostics of potential surrogate mother as to her psychological fitness and readiness for participation in the surrogacy motherhood program;
Complex medical examination of potential surrogate mother to confirm her fitness for extracorporeal fertilization, pregnancy and birth of a healthy child;
Primary medical advice, preparation of contract between genetic parents and surrogate mother with consideration of all possible aspects of individual situation of customers and introduction of the necessary clauses to the child-bearing contract;
Recommendations on choice of reproduction medicine clinic and other required medical institutions;
Services of translator during primary advice, and preparation of contract between genetic parents, and surrogate mother;
Coordination and the control of the surrogacy program, over the conclusion of the contract between genetic parents, and surrogate mother;
The IVF procedure and the period of medical pregnancy maintenance shall be paid for separately and shall not be a part of the package cost.

"Complex - 1" package deal – 19 200 USD:

The package deal "Complex - 1", includes all services of a package "Economical", plus time work of the curator on maintenance of the control and coordination of the program, (Without participation of the donor of egg)

Primarmedical advice, preparation of contract between genetic parents and surrogate mother with consideration of all possible aspects of individual situation of customers and introduction of the necessary clauses to the child-bearing contract;
Recommendations on choice of reproduction medicine clinic and other required medical institutions;
Services of translator during primary advice, and preparation of contract between genetic parents, and surrogate mother;
Coordination and the control of the program, over a birth of the child and reception of all necessary documents;
Indemnification to substitute mum for all term of pregnancy (payment of stage-by-stage-10 % on the fact of approach of pregnancy, 90 % 4-6 weeks prior to a birth of the child money resources are listed on the settlement account of substitute mother;
Indemnification to substitute mum if will be born more than one child;
The monthly maintenance (contents) for 9 months of pregnancy - is paid by the month;

Cost of the control and support of the program from the moment of the conclusion of the contract between substitute mother and genetic parents;
